Back to School Forms Due August 6!

At the start of each new school year, parents are required to review and update information pertaining to the student, parent(s), and emergency contact(s).  Parents and students are also asked each year to review and agree to the policies and procedures set forth by school administrators.  These new school year tasks are accomplished by completing the electronic BACK TO SCHOOL forms, which are now available through your parent PowerSchool account.  Remember, these forms are NOT available in the mobile app or by using your child's login credentials.

If you cannot remember your Username and/or Password, please use the link on the login page, Forgot Username or Password?  If you are trying to recover your Password, the Parent Username an Parent Email Address fields MUST match our system, otherwise, an email will not be sent.  Also, please check your spam or junk folder for the recovery email that is sent.  The Forgot Username option will ask for your email address associated with your parent account.  If no match is found, an email will not be sent.  If neither of these options help you log in to your parent account, please contact our PowerSchool Administrator by email for assistance.

After you login to your parent PowerSchool account, click the Forms link, located under the left Navigation menu.  BACK TO SCHOOL forms are listed at the top of the page.  Notice the Status of all forms currently reads Empty.  As you submit each form, the Status will change to Submitted.

Click on the first form A: Student Demographics.  Review each form and make any necessary changes.  Fields marked with a red asterisk (*) are required.  When done, click Submit at the bottom right-hand corner of the form.  A confirmation box will appear.  Click OK to load the next form.

If the residential address of the student listed at the bottom of the A: Student Demographics form is not correct, the Change of Address form must be completed.  This form is listed at the bottom under the Other category.

If you or your child are interested in receiving text message alerts and reminders, your child's cell phone number must be entered on the A: Students Demographics form; Parent cell phone numbers must be entered on the B: Student Contacts form.

On the B: Student Contacts form, please remember to review the Permissions assigned to each contact (Custody, Lives with, School Pickup, Emergency Contact).  The Custody checkbox should be selected for residential parents/guardians and non-custodial parents/guardians who are allowed contact with the child and are entitled to school information.  Click the blue pencil icon to the right of each contact record to review/update/delete information.  If multiple phone numbers are listed for a contact, school staff will call the number listed first.  If no response, the second number listed, if applicable, will be called, etc.

The health and safety of your child is of utmost importance to us.  In case of an emergency, it is imperative we have a list of medications your child takes to provide to first responders.  A comprehensive medical history and list of allergies are also required.  This information will be provided on the C: Health Information form.  Some medical conditions require additional paper forms to be downloaded and submitted to the school clinic.  Links to those documents are provided on the Health Information form.  If you have any questions, please contact our school health aide at 216-581-5771 when school resumes.

The G: Policy Acknowledgments form contains links to the Student Handbook and Chromebook Policy documents.  Both student and parent must electronically sign these forms, agreeing to abide by the policies and procedures set forth in these documents.

A New Year Reminders form will load at the end.  Please review these special reminders regarding the start of the school year.  When you return to the Forms page, make sure the Status of ALL forms under the Back to School section read Submitted.  Remember to select the Change of Address form at the bottom of the page ONLY IF THE STUDENT'S RESIDENTIAL ADDRESS (listed at the bottom of form A: Student Demographics) NEEDS TO BE UPDATED.

ALL BACK TO SCHOOL forms MUST be completed by Friday, August 6, otherwise, student will NOT be eligible to return to school or receive a class schedule and Chromebook.
